The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.A coalition of agencies in Baltimore City are working to prevent "excess disability" among low-income, community-based seniors through a program of integrated home intervention and ongoing support. After a comprehensive assessment, non-frail clients receive a Home Service Plan with home adaptations specific to their chronic health status and risk factors. Ongoing service coordination works to further prevent unnecessary accidents and disability and improve compliance with medical regimens. An estimated 76,000 senior households in Baltimore have incomes below $25,000, home adaptations and assistive devices are rarely subsidized and service coordination has been episodic and crisis driven. Funding partners include The France Merrick Foundation, The Erickson Foundation, and The Abell Foundation.
