The Chronic Care Networks for Alzheimer's Disease is a joint project of the Alzheimer's Association and the National Chronic Care Consortium that is intended to improve care for people with Alzheimer's disease and related dementias. The project involves creating strong linkages between the medical and home and community services and is being implemented currently in four sites. Foundation funding will support one-year development activities in three additional sites (Philadelphia, Minneapolis/St. Paul, and Upstate New York).
