The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.This project will be located at the Jonathan Lax Treatment Center, a community-based clinic. The grantee will establish a new disease management model for an HIV/AIDS population that is largely uninsured or dependent on public insurance. This model not only provides access to potentially promising new treatments, but also access to primary and specialty physicians with significant experience in HIV care. The Lax Center emphasizes culturally appropriate support with added emphasis on stabilizing patients affected by issues of substance abuse. Funding partners include the Philadelphia Foundation, Agouron Pharmaceuticals, the Philadelphia Health Care Trust, Roche Laboratories, Abbot Laboratories, and Bristol-Myers Squibb.
