This grant will support the evaluation of two projects Kaiser Permanente is undertaking in partnership with schools to improve health and health care for children. The first -- Healthy Minds -- will screen children referred from a school-based clinic who may be in need of mental health services. The Kaiser team will provide assessment, short-term intervention and triage to appropriate health, educational and mental health services in the community or school district. In the second -- Hearts in Power -- Kaiser will partner with several schools to undertake prevention activities in 6th grade classes targeting several risk factors for heart disease: tobacco use, physical inactivity, and high fat diets.
