The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.The site of this comprehensive project is the Alternative High School in Michigan City, Indiana, a "last chance" high school for students who have been expelled from traditional institutions. This School-Based Family Health Center will provide primary health care, mental health services, substance abuse services, and violence prevention training for young adults attending the school, as well as to the children of these young adults who are enrolled in the school's Early Childhood Development Program. In addition, home outreach services will be offered to all parents of students, and wellness programs will be offered to the community. Anticipated outcomes include increased school attendance, improved grades and self-esteem, reduction in disciplinary problems, and improved health and well being of participants. Funding partners include the Aetna Foundation, Memorial Health System, Inc., Barker Welfare Foundation, and Michigan City Area Schools.
