Planning for a population health improvement leadership conference

This grant supports planning activities for a leadership conference on population health that focuses on defining/measuring population health, understanding the determinants of population health, and identifying steps that can be taken to improve population health status. During the nine-month planning period, the project team will: conduct a review of current activity relating to population health improvement effects; establish a planning committee, including federal government officials, leaders from managed care plans, and representatives from RWJF's medicine and public health initiative; and produce a conference plan that identifies topics to be covered, invitees, and follow-up activities.
