The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.Under this grant, Providence Hospital will develop and operate the Perry Family Health Center (PFHC) to be located within The Perry School Community Service Center in a severely disadvantaged community in Washington, D.C. The Center, created and managed by a grassroots coalition, houses a strategic blend of services including dental care, job training, food and housing assistance, and child care. As a centerpiece of the design, PFHC will provide primary health and prevention services in a private practice model to complement these services. This collaborative effort brings together local residents and a major health care provider to improve a neighborhood's health and social welfare.
