This research project extends to one year (from six months) the data collection period for a study that is currently being conducted on the effectiveness of a nurse practitioner primary care practice in managing three common chronic diseases. The population served is a predominantly Hispanic Medicaid population in the Washington Heights section of New York City. The clinic is one of five ambulatory care clinics of the Columbia Presbyterian Medical Center. Current funding for the evaluation is being provided by the New York State Health Department, the Department of Health and Human Services Division of Nursing, and the United Hospital Fund. Foundation funds will be used to carry the project to the end of 1998 in order to leverage funds from other sources for the last year of the study.
