The Foundation's Medicare/Medicaid Integration Program is a state demonstration to test the operation and design of delivery systems that integrate long-term and acute care services under combined Medicare and Medicaid capitation payments for elderly patients. The objectives are to promote integration of acute and long-term care services in managed care plans, create health plan accountability for costs and outcomes, and reduce incentives for cost shifting between Medicare and Medicaid.Six states make up the New England Consortium under this program (Connecticut, Maine, Massachusetts, New Hampshire, Rhode Island and Vermont). The goal of the consortium is to work in partnership to develop strategies to integrate the financing and delivery of health care and ancillary social services. The New England Consortium will be centered at the University of Southern Maine's Muskie School of Public Service. Coordination efforts are supported as part of this grant to the State of Maine.
