This is a project to evaluate ElderHealth, an innovative managed care program that serves "dual eligibles" (persons eligible for both Medicare and Medicaid) in low-income areas of Baltimore. ElderHealth takes full responsibility for the delivery of covered services to this group under contract to Blue Cross/Blue Shield of Maryland's Medicare HMO. The evaluation will compare ElderHealth enrollees with fee-for-service, dual-eligible individuals from similar neighborhoods, and with individuals profiled in the national Medicare Current Beneficiaries Survey. The investigators will examine the process and outcomes of care, including health and functional status, total costs, rates of utilizations, and patients' perceptions and satisfaction. There is strong public policy interest in evaluating whether ElderHealth achieves its multiple goals, as it represents a potential model of integrated care that addresses the high cost associated with these individuals.
