The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.People with chronic mental illness and chemical abuse are at high risk for multiple arrests and incarceration. Under this forensic psychiatry project, the grantee will create linkages between appropriate mental health and social services and establish a case advocacy team to provide a therapeutic approach to this problem. Comprehensive and coordinated services -- including transitional housing -- will be provided to help these individuals improve their lives and avoid incarceration. Funding partners include Coordinated Care Services, Inc., and Monroe County Office of Mental Health.
