Assessment of efforts to provide Comprehensive management of chronic renal disease

The development of innovative approaches to manage chronic illness has become an important health issue, particularly given the expected increase in coming decades in the number of individuals with chronic diseases. Under capitated reimbursement, managed care providers have begun to focus on strategies to deliver health care services in a cost-effective manner. The goals of this project are: (1) to evaluate cost and quality outcomes for chronic renal patients treated in a managed care setting; and (2) to document the set of clinical interventions used to achieve the outcomes. When chronic renal disease patients reach the end stage renal disease phase, they require high-cost, ongoing treatment. The focus will be on patients treated at Los Angeles Medical Center, a practice site of Kaiser Permanente's Southern California Region. Patient outcomes will be compared to those experienced by other sites and to patients treated outside of the Kaiser health care delivery system.
