Program to provide emergency and short notice home care workers for persons with disabilities

The Foundation's program, Independent Choices: Enhancing Consumer Direction for People with Disabilities, was designed to foster the development of consumer-directed home and community-based services for people of all ages with chronic disabilities.This is one of 11 grants awarded under the Independent Choices program. The purpose of this project is to provide a cost-effective, consumer-driven system for providing rapid response emergency services for In-Home Supportive Services (IHSS) consumers living in Oakland, California. Under the IHSS program, consumers are responsible for locating, hiring, supervising, and firing their own workers. Despite being preferred by consumers, this independent provider mode sometimes falls short in meeting short notice or emergency needs (e.g., when there is an exacerbation of illness or an injury, or when their regular worker is ill or unavailable). Inability to locate a replacement worker can force consumers to seek more expensive care from 911, an emergency room, or a physician. This project will: assess the need for rapid response services; develop a model program; test the model by delivering a range of effective, rapid response, short-notice, and emergency in-home services for Oakland IHSS consumers; evaluate the model; and develop a strategy to sustain the service over the long term.
