The Foundation's Medicare/Medicaid Integration Program is a state demonstration to test the operation and design of delivery systems that integrate long-term and acute care services under combined Medicare and Medicaid capitation payments for elderly patients. The objectives are to promote integration of acute and long-term care services in managed care plans, create health plan accountability for costs and outcomes, and reduce incentives for cost shifting between Medicare and Medicaid.This contract provides consulting services and technical assistance to states under the Foundation's Program to Promote Long-Term Care Insurance for the Elderly, State Initiatives in Long-Term Care, and Medicare/Medicaid Integration programs. This work will complement technical assistance and direction of the National Program Office.
