The Foundation's program, Promoting Excellence in End-of-Life Care, was designed to foster long-term changes in health care institutions to substantially improve care for dying persons and their families.This grant supports technical assistance and direction for this Foundation program. The National Program Office will provide leadership to this program which will fund projects of national significance in improving end-of-life care. As part of this endeavor, the program will construct a resource sharing network on best practices in end-of-life care within health care institutions; convene regional and national meetings to give visibility to the need to improve end-of-life care; develop communication strategies to increase the public's expectations about care at the end of life; help coordinate recommendations emerging from the Last Acts Campaign; and integrate the national program's activities with other Foundation grants to improve end-of-life care.
