Medicaid/Medicare Integration Project

The Foundation's Medicare/Medicaid Integration Program is a state demonstration to test the operation and design of delivery systems that integrate long-term and acute care services under combined Medicare and Medicaid capitation payments for elderly patients. The objectives are to promote integration of acute and long-term care services in managed care plans, create health plan accountability for costs and outcomes, and reduce incentives for cost shifting between Medicare and Medicaid.Within a managed care environment, the State of Colorado will implement an acute and long-term care integration project, evaluate the results, and establish the groundwork for replication in Colorado. Collaborators include the Colorado Medicaid Program, Rocky Mountain HMO, and Mesa County Options for Long-Term Care. By integrating the funding mechanisms under Medicare and Medicaid, the objective is to provide cost-effective preventative and community-based care, while avoiding unnecessary hospital and nursing facility admissions. This implementation project is built upon an earlier planning grant funded by the Foundation under the State Initiatives in Long-Term Care.
