Defining home care risk groups: A strategy for improved effectiveness and efficiency

The Foundation's Home Care Research Initiative was designed for researchers and policy analysts to explore key issues in the area of home and community-based care for the chronically ill.This study is designed to provide program managers with tools for sorting patients into groups based upon their risk of specific adverse outcomes and the degree of the severity of those outcomes. Protocols will be designed to mitigate specific risks, and pilot testing will be conducted on real patients in a home care population.
