This grant supports a study to evaluate the implementation of the Agency for Health Care Policy and Research Smoking Cessation Guideline in a large health care organization. Allina Health System will evaluate full guideline implementation in 20 of its contracted Independent Practice Association (IPA) clinics using a randomized trial design, with 10 clinics serving as pilot clinics and 10 clinics serving as controls. Data will be collected through patient exit surveys, chart abstraction, claims data, and a follow-up survey. The investigators will compare both outcome measures (quit rates) and process measures (fidelity of guideline implementation) across pilot and control clinics. It is expected that study findings will help to answer important questions regarding the means by which full guideline implementation can be achieved in large health care organizations and the impact that such implementation would have on tobacco use.
