Technical assistance and direction for RWJF's Chronic Care Initiatives in HMOs program

The Foundation's program, Chronic Care Initiatives in HMOs, was designed to identify, nurture, and evaluate innovations in the delivery of services to chronically ill patients in prepaid managed care organizations.This is the final grant to provide technical assistance and direction under the Foundation's Chronic Care Initiatives in HMOs. During this last eight-month period, the major priorities are: (1) information dissemination through publications and meetings; (2) grants monitoring; and (3) as research becomes available, identification of opportunities to promote adoption of worthwhile initiatives.
