Community-based primary health care services for people with chronic mental illnesses

The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.The grantee, in conjunction with the East Orange General Hospital and the VNA of Essex Valley will design, implement, and evaluate a primary care delivery model for people with severe and persistent mental illness who are living in community residential facilities and boarding homes in East Orange. A certified nurse practitioner and a registered nurse will provide on-site primary care as well as coordination and referral services to 250 mentally ill individuals. The grantee will collect and analyze data on medical problems and utilization patterns; determine the model's impact on physical, emotional and social functions; and assess cost effectiveness.
