Hospice program for patients who live alone

The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.Under present hospice guidelines, patients who do not have a resident caregiver to tend to their daily needs are not eligible for services. To respond to this impediment to patients living alone, the Solo Program utilizes a social worker who works with the patient's physician and the interdisciplinary team of hospice professionals and who serves as care manager to coordinate daily needs of the patients (meals, chores, volunteer services, etc). A pilot program found that quality of life was improved and medical and hospital expenses were greatly reduced. The grantee expects to provide hospice care at home to 100 patients daily and to develop an educational program to inform the public, physicians, and health professionals of the benefits.
