The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.This project is a collaborative community effort to provide comprehensive health care for underserved and unserved adolescents, provide disease prevention and health promotion/education services to adolescents and their families, and to record, maintain and assess data on adolescent health needs and the impact of the Center's services on the community. The Center will provide coordination and referral for school-based care, and will also serve dropouts who comprise 12 percent of the adolescent population. The grantee estimates that the free-standing health center will experience 6,000 visits per year.
