Health Tracking

The Foundation's Health Tracking initiative was designed to support efforts to understand better health system changes and how these changes affect people.Under this program contract, the Center for Studying Health System Change will implement a variety of activities designed to be the keystone of the Foundation's Health Tracking efforts. Principal among these activities will be overseeing the Community Tracking Study, an ambitious effort to track selected indicators of health and health care in 60 communities across the country, including 12 that will be followed intensively. These activities will begin implementing the heart of the Foundation's Health Tracking efforts.
