Project for long-term supportive services for developmentally disabled residents of New Hampshire

The State of New Hampshire will make a significant transition in its system of long-term care for persons with developmental disabilities which is funded largely by the Medicaid program. The State will undergo a transition from a system of traditional programs and services to one that allocates resources according to individual needs and preferences, and simultaneously rebuilds a network of informal supports. They plan to replicate the Foundation-supported Monadnock Self-Determination pilot in 9 of the State's 12 regions over a 3-year period -- funded under the Foundation's Building Health Systems national program and has demonstrated significant cost savings and high satisfaction among participants. New Hampshire is on the cutting edge of this movement, and other states are proceeding in a similar direction. The project will use the flexibility of an individual budget to more effectively allocate resources. Cash vouchers, which require additional waiver authority from HCFA, will not be a focus of this project. Foundation support will be used primarily for staff at the state and local level to lead and facilitate the significant changes that need to occur in the transition from a system that provides a menu of services to a system that finds creative solutions to the individual needs of each client.
