The Foundation's program, Chronic Care Initiatives in HMOs, was designed to identify, nurture, and evaluate innovations in the delivery of services to chronically ill patients in prepaid managed care organizations.This is a unique project focusing on treatment of Medicaid eligibles with substance abuse and mental health problems in a prepaid environment. During this planning phase, the grantee will develop a comprehensive program to bridge social, health, parenting and occupations resources for AFDC women diagnosed with alcohol/substance abuse problems, mental health problems, or both. Working with local agencies, a targeted case manager will assess the family's needs and develop a long-term treatment plan incorporating a wide range of services including detoxification, drug treatment, psychological therapy, parenting skills, respite child care, legal assistance, literacy building, vocational skill development, and transportation assistance.
