The Foundation's program, Chronic Care Initiatives in HMOs, was designed to identify, nurture, and evaluate innovations in the delivery of services to chronically ill patients in prepaid managed care organizations.The purpose of this project is to evaluate the Identification and Early Intervention program in an independent practice association setting to improve primary care for patients whose chronic conditions place them at risk for disability or hospitalization. The program includes screening to identify at-risk patients, and improved communication between physician offices and the community agency providing case management through use of a Geriatric Resource Person. It is expected to improve care and reduce overall costs.
