Effect of physician gatekeepers on health care cost, access, and quality
The Foundation's Changes in Health Care Financing and Organization program was designed to support research, demonstration, and evaluation projects examining major changes in health care financing.The purpose of this project is to evaluate the impact of a major component of managed care--the use of a primary care physician as a gatekeeper to authorize all health services of HMO members. By analyzing pre-and post-data from Massachusetts General Hospital (MGH) employees selecting the MGH Plus insurance plan, the researchers will determine the effect of using a gatekeeper approach on costs, access, and quality of care, as well as on patient and provider satisfaction. In addition, they will analyze economic, demographic, and other factors that may influence employees to choose a health care plan that uses a primary care physician as a gatekeeper. The objective of this study is to provide policy makers with new evidence of the effectiveness of a key component of managed care.
