The Foundation's program, All Kids Count: Establishing Immunization Monitoring and Follow-up Systems, was designed to support the development and implementation of monitoring and follow-up systems to improve and sustain access to immunizations for preschool children.The purpose of this project is to develop a statewide centralized immunization and tracking program within New Jersey. The grantee will collaborate with the New Jersey Department of Health to: (1) pilot test an immunization registry, tracking and follow-up (RTF) program in Camden that receives and integrates immunization record information from AFDC and WIC programs, community health centers, hospitals and private physician practices; (2) expand local RTF programs to four other large cities; (3) develop and implement a statewide immunization registry; and (4) provide public and professional information and training about the registry. If successful, this program will increase immunization coverage of preschool age children in target communities to 90 percent.
