The Foundation's initiative, Building Health Systems for People with Chronic Illnesses, was designed to find models of caring for people with chronic illnesses that will overcome the fragmentation, financing barriers, and episodic care of the current system.This grant provides continued technical assistance and direction for this program. This year's activities will include the completion of the first set of awards, a second round of grants, and marketing of the program. The specific objectives of the grant include: (1) continue review process for round one -- budget reviews, site visits, etc.; (2) revise program announcement for round two; (3) invite applications and review next cycle of proposals; (4) monitor grantees' progress and provide technical assistance as needed; and (5) refine the conceptual framework concerning systems development for chronic illnesses and assist in the dissemination of findings from the projects.
