The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.The purpose of this project is to establish a central intake and referral system and shared provider pool between and among nine community health clinics and two public health agencies. This project is a collaborative effort of nine neighborhood clinics and two public health agencies serving low-income residents of Ramsey County, Minnesota. The network supported by the grant will develop a centralized intake and referral system, initiate a coordinated outreach effort, and establish a shared provider pool committed to the practice of community medicine. Family Tree Clinic enjoys a broad base of community and institutional support. Key public officials are vested in the success of the innovation and have expressed a willingness to sponsor legislative support. Funding partners include F.R. Bigelow Foundation, Mardag Foundation, the St. Paul Company, and the St. Paul Foundation.
