All Kids Count

The Foundation's program, All Kids Count: Establishing Immunization Monitoring and Follow-up Systems, was designed to support the development and implementation of monitoring and follow-up systems to improve and sustain access to immunizations for preschool children.This is one of 23 one-year planning grants under the All Kids Count program. The main goal of the program is to establish immunization monitoring and follow-up systems in an effort to increase immunization rates among preschool children and reduce rate of illness, disability, and death from vaccine-preventable diseases. The objective of this project is to implement a system to link children and families to already available resources and to establish institutional accountability to provide service to them. Specific activities will include: (1) establishing the management structure necessary to implement the system; (2) implementing a prototype computerized registry in Cleveland; (3) design operating protocols and procedures as well as agreements between city officials; and (4) linking the All Kids Count project to Healthy Family Healthy Start program in Cleveland. A tracking system will be developed to identify and follow-up children with missed immunizations. Issues of confidentiality will be addressed. An education and marketing strategy to keep the public and providers informed will be developed.
