Project to help rebuild New York City's primary care infrastructure
The purpose of this project is to organize a multiple foundation partnership that will direct and manage the development of five significant primary care programs to better serve the predominately low-income and minority-resident disadvantaged New York City communities. The project will serve a minimum of 20,000 clients/year and result in an increased service capability of at least 100,000 visits/year in South Bronx, Jamaica, Bushwick, and Harlem. The five primary care service programs will develop different service models tailored to the needs of the communities served by the project. In addition to the five centerpiece projects, the grantee will provide technical assistance to other health care providers and community organizations developing primary care programs in New York City. The grantee will play a critical role in areas of project design, development, management and evaluation.
