Homeless Families Program

The Foundation's Homeless Families Program was designed to enable communities to develop the infrastructure necessary to strengthen the potential of homeless parents and their children to function successfully.The City of Baltimore's project goal is to improve the City's ability to effectively and efficiently coordinate the health care and supportive services delivery systems in Baltimore for homeless families. Specifically, the grantee will: (1) work with the National Program Office and Federally Qualified Health Centers consultant, develop a system among selected 330s that will provide primary health care to homeless families and finance case management; (2) enroll 290 families in the Homeless Families Program and expand enrollment in the project as case management capacity grows; (3) supervise case management staff; (4) ensure that families receive supportive services from Family Support Centers and Family Development Centers; (5) subcontract with the YWCA to operationalize a family mentoring program; and (6) collaborate with the PHA's Housing Relocation Office (central intake) and the Division of Family Services to provide supportive services for enrolled families living in public housing.
