The Foundation's initiative, Building Health Systems for People with Chronic Illnesses, was designed to find models of caring for people with chronic illnesses that will overcome the fragmentation, financing barriers, and episodic care of the current system.This grant enabled the grantee to provide start-up technical assistance and direction for this Foundation national program. The goals and objections of this project are for the Program Director to: (1) set up a Program Office and purchase office equipment and supplies; and (2) develop an Request for Proposals.
