The Foundation's Community Care Funding Partners Program is an eight-year matching grant program designed to encourage community and regional corporations and foundations to support small-scale local health centers serving the medically indigent.This renewal of the Community Care Funding Partners Program supports adolescent health care centers in four schools in the Washington Heights section of New York City. Goals for the three-year grant include providing physicals to 1,600 students; providing first aid and primary episodic care for approximately 8,000 visits; and providing individual and group social work services to 1,500 students for a total of 8,000 visits. Nutrition and fitness programs will be offered; pregnancy prevention interventions will be strengthened; work programs will be expanded; and surveys will be conducted to assure client satisfaction.
