Development of rural community-based systems for long-term care
The purpose of this project is to promote the development of rural community-based systems of long-term care. To meet this goal, the grantee will: (1) establish the criteria for site selection; (2) establish criteria whereby grants and loans will be made available to select sites for model development; (3) design a process whereby technical assistance will be provided to help sites design their projects and apply for financing; and (4) design a process for marketing the program to state offices of rural health and other appropriate rural organizations which will help identify and develop potential rural communities.
