Network of local service centers for high-risk youth

The Foundation's program, Local Funding Partnerships, was designed to continue a matching grants program to establish partnerships between RWJF and local grant makers in support of innovative, community-based projects that improve health and health care for vulnerable populations.This grant represents renewal funding for the Lemmon Avenue Youth Impact Center for high-risk adolescents. This service delivery system, funded through the Local Funding Partnerships, co-locates 22 Dallas area agencies that provide health and human services to troubled youth. The program improves access to care by improving coordination of existing programs by locating them in a neighborhood center that offers case management. Data will be collected for process and outcome assessment of the program. The project will continue to coordinate and facilitate care for over 400 youth. Health, mental health, dental, and social services will be provided on site. Education and prevention programs, recreation, crisis intervention, and counseling programs will be provided. Bridge staff will provide case management, outreach services, information, and referral and on-site group educational training. At the end of the grant period, it is anticipated the project will have a stable funding base from local and state agencies. Funding partners include: the Meadows Foundation, Communities Foundation of Texas, Dallas Foundation, Leland Fikes, Hillcrest, Hoblitzelle, Rosewood, Southwest Osteopathic Physicians and Southwestern Foundation.
