Improving Service Systems for People with Disabilities

The Foundation's program, Improving Service Systems for People with Disabilities, was designed to strengthen the capacity of community-based agencies run by and for people with physical disabilities to create comprehensive systems of services for this population.Under this grant, the Center for Independence of the Disabled in New York will undertake the planning necessary to: (1) contract for a market survey of unmet needs; (2) develop a primary care system to serve people with disabilities, with heavy reliance on nurse practitioners, and on linkages with acute care facilities; (3) establish an advisory council for the health care service project; (4) file a CoN; (5) develop financing strategies, primarily through third-party reimbursement; and (6) integrate this activity with other services provided by the Center. The Year I products will include a market or needs assessment and an implementation plan.
