The Foundation's program, Local Funding Partnerships, was designed to continue a matching grants program to establish partnerships between RWJF and local grant makers in support of innovative, community-based projects that improve health and health care for vulnerable populations.This grant supports a Local Initiative Funding Partner Program to improve pregnancy outcomes and infant care in two of Philadelphia's poorest neighborhoods by basing outreach and case management teams in the area clinics which provide free maternity and other health services. Each team consists of a nurse and four lay health workers recruited from the community who will seek out all pregnant womenn not receiving adequate care, arrange appointments and transportation to the health center, be present at the client's visit and follow up on her status through home visits twice a month. The team nurse will provide clinical care to the mother. Funding partners include: the Fels Fund, Philadelphia Foundation, Scott Paper Company Foundation, Douty Foundation, W.W.Smith Charitable Trust, and the Henrietta Tower Wurts Memorial.
