School-Based Adolescent Health Care Program (Lincoln & East High Schools)
The Foundation's School-Based Adolescent Health Care Program was designed to strengthen the capacity of communities and families to meet the multiple health and health-related needs of adolescents by utilizing local public secondary schools as a principal locus of care.This is a 2nd-phase grant awarded under the 19-site national program, to provide comprehensive health services to the 3,288 adolescents at the East/Manual site and 1,898 adolescents at the Lincoln site. Clinics are staffed by a full-time NP, mental health practitioner, secretary/medical technician and project director; half-time physician. Clinic staff will be shared by the two clinics. Each student will designate a primary care provider outside the clinic. An option list has been developed for those with no provider.
