Northwestern Community High School adolescent health center

The Foundation's Community Care Funding Partners Program is an eight-year matching grant program designed to encourage community and regional corporations and foundations to support small-scale local health centers serving the medically indigent.This represents a renewal of a Community Care Funding Partners Program grant for a primary care health center at Northwestern High School. Services began in 1984 under the original grant. The objectives are to: provide early detection and treatment of conditions and illnesses; motivate teenagers to adopt healthy lifestyles and discard risk-taking behaviors; and teach adolescents how to use the health care system appropriately and effectively. The clinic is staffed by a full-time nurse practitioner, RN/health educator and certified medical assistant and a part-time psychiatric social worker. Medical backup is provided by a physician serving as medical director plus specialists, equivalent to about 20 percent of a full-time doctor. Subspecialty referrals and hospitalizations, including deliveries, are provided by Hurley Medical Center, which also employs and supervises the on site staff and operates the practice as a satellite. The clinic expects to serve over 1,300 clients, with expected visits to total over 2,700 annually. Matching funds will be provided by Mott Children's Health Center and the Hurley Medical Center.
