Program to detect and prevent high-risk health behaviors among youth

The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.Under this project, the public schools in Greater Kansas City and the area's health, mental health, and other youth service institutions will participate in a coordinated program to reach adolescents at an early age and instill them with the capacity to avoid behaviors inimical to their health. The program will put in place a process for continuous education, surveillance, counseling, and referral for all students in the participating schools, plus a home and school case management program for students at high risk. The project will depend heavily upon the resources of the participating health care institutions. They bring to the project assurance of prompt and appropriate medical care not present in other adolescent health programs involving the public schools, except for the RWJF School-Based Adolescent Health Care Program. The participating health institutions include three such clinics, neighborhood health centers, and the health and mental health departments of Metropolitan Kansas City.
