The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.This grant supports establishment of a residential facility for homeless AIDS patients. The program will provide a continuum of care including case management, home health aide and nursing services, transportation, social services and counseling. Located in the Coconut Grove area, the site is on the grounds of Mercy Hospital which will be a provider for inpatient and outpatient services. Jackson Memorial Hospital will also be utilized. The program will be ecumenical with no religious qualifications for acceptance. A cadre of volunteers will participate in the social and recreational programs to be provided. Residence at the apartment complex will not be a requirement for continuation in the program should a client find a home with family or friends or become hospitalized. Parent/child dyads with AIDS could be accommodated.
