The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.This grant supports the development of a case-management approach to provide nonacute care services to people with AIDS using a day treatment program, home care services, and a skilled nursing facility. Backed up by three hospitals, this program will be the first full-service AIDS care system in New York City.
