The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.This grant supports a program for pregnant and parenting teens in Southwest Idaho to decrease the low birthweight rate among adolescent mothers, increase the rate of school continuation or employment of young parents, and decrease the incidence of repeat births using a case-manager approach. Services will be provided to more than 100 pregnant adolescents and/or mothers, 50 infants and children, and 60 partners until the infant is two years old.
