Primary health care for adults with chronic disabilities

The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.The Family Practice Center, a non-profit group practice, is a cooperative effort of the Department of Family Medicine, UMDNJ-RWJ Medical School, and St. Peter's Medical Center. Under this grant, the center will provide primary health care to adults with chronic disabilities, including both developmental disabilities and chronic mental illness. The objectives are: (1) to provide personalized primary health care to 200 disabled clients that will maintain or improve their health status and functional capabilities; and (2) to demonstrate that a capitated managed care model for the disabled can be developed. The Program will work in cooperation with Medicaid's Garden State Health Plan.
